Introduction - about North Halifax Community Wellbeing Partnership

North Halifax Community Wellbeing Partnership is a multi-agency group of partners. It includes primary care, community nurses, North Halifax Partnership and Public Health. As a partnership, we are working together to help everyone live healthier, happier lives.

Help us to improve heart health support

Do you live in North Halifax or do you use a GP practice there? If so, we want to hear how we can offer you the best support to keep your heart healthy.

What is heart health?

• Heart health or cardiovascular health refers to the condition of your heart and blood vessels.
• There are things we can all do to manage and improve our heart health and prevent serious problems like strokes and heart attacks.
